The cushion is not an enclosed ring, but rather has an opening at the front that allows for adjustments to the actual diameter of the cushion. There is a clip that will help maintain the diameter of the ring so when sitting the cushion does not spread further than you may want it. Initially the cushion appears very thick, but once you sit on it it compresses very rapidly. I am almost tempted to get two. If one of the uses is to help with a hemorrhoidal issue, well I suppose it is better than a harder surface, but only barely.. The cushion compresses too much for my liking. Would I buy this again? No.
